What necessary paperwork or documents do I need for enrollment ?

• Copy of GED, High School Diploma or College Transcripts
• Copy of Driver's License
• Copy of Childhood Immunizations or Disease Dates of: Mumps, Measles, Rubella, Chickenpox
• Proof of Hepatitis B Immunization
• Proof of Tuberculin Test
• For Veterans, Proof of Eligibility for Benefits
• For Veterans, Form 1995
